A letter from Dr. Breaux

Dear Patients:

It is with mixed emotions that I am announcing my departure from Brentwood Pediatrics, effective April 19, 2019. It has been a great pleasure providing for your children’s health care needs over the past years, and it is not easy for me to give it up.

As of April 19, 2019, Rebecca Jeannine Bell, CPNP will be taking over my practice. I am pleased that you will have the opportunity to have her as your provider. Rebecca Jeannine Bell, CPNP is a well‑trained graduate of Vanderbilt University. While attending Vanderbilt she worked as a registered nurse at a local pediatric primary care clinic. In 2017, she graduated Summa Cum Laude with an M.S. in Nursing, and a concentration in pediatric primary care. Jeannine is a member of the National Association of Pediatric Nurse Practitioners and the Cumberland Pediatric Foundation. I am glad to be leaving my patients in her capable hands. I am working with Rebecca Jeannine Bell, CPNP to make this transition a smooth one by helping her become familiar with you and your family. Of course, you may seek medical care from another provider if you like. If you choose to do so, I recommend looking for a new provider as soon as possible.

Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only through your permission. If you choose to see a different physician, please make sure to sign an authorization form at our office as soon as possible so we may transfer your records to your new doctor. Until then, your records will remain on file with Brentwood Pediatrics.

I have truly valued your relationship and your trust while caring for your children. Best wishes for your future health.

Lori A. Breaux, M.D.
